== Molecular problem: fibrosis == When faced with pressure overload, the heart attempts to compensate with a number of structural alterations including hypertrophy of cardiomyocytes and increase of extracellular matrix (ECM) proteins. Rapid accumulation of ECM proteins causes excessive fibrosis resulting in decreased myocardial compliance and increased myocardial stiffness. The exact mechanisms involved in excessive fibrosis are not fully understood but there is evidence that supports involvement from local growth factors FGF-2, TGF-beta and platelet-derived growth factor. TGF-β1 plays an important role in cardiac remodelling through the stimulation of fibroblast proliferation, ECM deposition and myocyte hypertrophy. The increase in TGF-beta 1 expression in a pressure-overloaded heart correlates with the degree of fibrosis, suggesting TGF-beta 1 involvement in the progression from a compensated hypertrophy to failure. Through an autocrine mechanism, TGF-beta 1 acts on fibroblasts by binding TGF-beta 1 receptors 1 and 2. == History == Salvia divinorum has been used as an entheogen by the Mazatec people of Mexico for hundreds of years. The American anthropologist Jean Bassett Johnson made expeditions to Mexico in the mid-to-late 1930s, observed the entheogenic use of Salvia divinorum by the Mazatecs there, and was the first to describe the existence of the plant in 1939. Subsequently, other researchers, including Blas Pablo Reko and Robert J. Weitlaner, also described the plant and its use in the 1940s and 1950s. Arturo Gómez-Pompa classified the plant as belonging to the genus Salvia in 1957, but was unable to completely identify it at the time due to absence of flowering material. Finally, Robert Gordon Wasson and Albert Hofmann collected flowering specimens of the plant in the early 1960s and sent them to Carl Epling, the leading expert on the Salvia genus of the time, who defined the plant as a new species named Salvia divinorum in 1962. Salvinorin A was isolated from Salvia divinorum and identified by Alfredo Ortega and colleagues in 1982. They used a combination of spectroscopy and X-ray crystallography to determine the chemical structure of the compound, which was shown to have a bicyclic diterpene structure. == Mechanism of action == Nalfurafine is an orally active, centrally acting, highly potent, selective full agonist of the κ-opioid receptor (KOR) (Ki = 75 pM; EC50 = 25 pM). As touched on above, nalfurafine shows atypical properties as a KOR agonist relative to other drugs. Notably, it does not completely substitute for the prototypical KOR agonist U-50488 in rodents, indicating qualitative differences in the discriminative effects of the two compounds. Moreover, unlike U-50488, it produces neither conditioned place aversion or preference in rodents. The drug is a 4,5-epoxymorphinan derivative, and is structurally unique relative to other KOR agonists. Nalfurafine may be a biased agonist of the KOR or a KOR subtype-selective agonist. Indeed, it has been found to act as a biased agonist of the KOR, preferring activation of β-arrestin signaling in vitro, but paradoxically, β-arrestin appears to be responsible for KOR agonist-induced aversion, and nalfurafine furthermore shows paradoxical effects in vivo that are not consistent with its in vitro profile. As such, more research is needed to clarify the distinct mechanisms and effects of this drug. Nalfurafine has been found in vitro to bind to the μ-opioid receptor and to possess weak partial agonist activity at this site, albeit with much lower affinity relative to the KOR. However, in vivo, nalfurafine has shown no indications of MOR agonism or antagonism in animals or humans, including no evidence of rewarding or reinforcing effects or physical dependence.

=== Dependence and withdrawal === Physical dependence can develop with long-term clonidine use, and abrupt discontinuation can cause a withdrawal syndrome marked by a pronounced rebound increase in blood pressure. Symptoms of clonidine withdrawal include a marked rise in blood pressure with symptoms such as headache, sweating, insomnia, agitation, tremor, palpitations, nervousness, and nausea. Among dependent individuals, the severity of drug withdrawal appears to be significantly more pronounced in people with pre-existing hypertension and after prolonged treatment at higher doses (i.e., >900 mcg/day), and severe cases (hypertensive encephalopathy, stroke, and death) have been reported albeit rarely. To emphasise that there was now only one Indian Army, and that all units were to be trained and deployed without regard for their regional origins, the regiments were renumbered into single sequences of cavalry, artillery, infantry of the line, and Gurkha Rifles. Regimental designations were altered to remove all references to the former Presidency Armies. Where appropriate subsidiary titles recalling other identifying details were adopted. Thus the 2nd Bengal Lancers became the 2nd Lancers (Gardner's Horse). The new order began with the Bengal regiments, followed by the Punjab Frontier Force, then the regiments of Madras, the Hyderabad Contingent, and Bombay. Wherever possible a significant digit was retained in the new number. Thus the 1st Sikh Infantry became the 51st Sikhs, the 1st Madras Pioneers became the 61st Pioneers, and the 1st Bombay Grenadiers became the 101st Grenadiers. The Gurkha Regiments had developed into their own Line of rifle regiments since 1861. They were five of these until they were joined by the former 42nd, 43rd, & 44th Gurkha Regiments of the Bengal Army, who became the 6th, 7th, & 8th Gurkha Rifles. The numbers 42, 43, & 44 were allocated respectively to the Deoli and Erinpura Irregular Forces and the Mhairwara Battalion from Rajputana.
